Year of Care
The Year of Care Model © has been developed by Professor Pieter Degeling and colleagues at the Centre for Clinical Management Development Unit (CCMD) in the School for Health at Durham University.
The model is a planned, comprehensive approach to assisting people with long term conditions to self-manage their care over an appropriate length of time and within limits appropriate to the stage of disease progression, with the goal of maximising their wellbeing, quality of life, and the efficient use of healthcare resources. The Year of Care model (YoC) is used to systematise the design, commissioning and delivery of services to people with long term conditions as well as informing the 'time specified' care plan that they have negotiated with primary care based service providers. The Year of Care Program is underpinned by hte concept of co-production which clarifies the rights and relationships of people with long term conditions, families, communities, and organisations.
Primary Care Trusts around the UK are developing their own Year of Care Programs with people with a variety of conditions including COPD and diabetes.
The aim of this site is to create a central resource of those involved in the Year of Care concept and debate. The forums will allow discussion about individual experiences, aspects of the care models and concepts for development.
